PRESCRIPTION, RECEIPT REQUEST:

To get a copy of your current Rx or receipt, you may text (540) 300-2771 or email info@geteyeexam.com . Use "Rx Request" or "Receipt Request" in the email subject line. Please include your name, date of birth, which Rx, and where to send it. Request must be made by the patient unless he is a minor. Please note that we do not release prescription if your exam was more than a year ago as it has already expired. Please allow up to 15 days to complete.

We do not accept requests at the front desk.
